MONTHLY PERFORMANCE REVIEW 

RATING INSTRUCTIONS GUIDELINE/BANDING SYSTEM Describe the employee's contributions in each of the performance categories below. It is IMPERATIVE that you illustrate specific, detailed examples since the last performance evaluation. Ratings MUST support and be substantiated by narrative comments.

Competency and Rating 

Quality {Completes high-quality work according to specifications.  Thoroughly follows standards and procedures. Keeps complete records. Pays attention to details. Has a strong sense of quality and knows how to achieve it.}

Productivity {Completes work within the allocated time & cost and meet or achieves deadlines with profits. Always positive about work and achieving deadlines.}  

Communication Skills {Organizes and expresses ideas and information clearly, using appropriate and efficient methods of conveying the information.}

Interpersonal Skills {Is sensitive to the needs, feelings, and capabilities of others. Approaches others in a non-threatening and pleasant manner and treats them with respect.}

Team-working and Development of others {Works on projects as part of a team, exchanging ideas and contributing skills that complement those of the other team members. Fulfills commitments to team members.}

Process Initiative & Improvement {Strives to learn and improve. Seeks out ways to better themselves and the company. Takes on responsibilities. Does not remain idle – is hard working. Self-motivated.}  {Continually looks for ways to improve and promote the process. Applies feedback to improve performance and process}

Planning and Target Achieved {Possesses skills and knowledge to perform the job competently with proper planning.}

Office Discipline {Punctuality, Leave Management, Reporting, No. of Breaks, Attire etc}

Timesheet Reporting {Timesheet reports being a strategic tool for evaluating business success and planning. Punctuality in filling timesheet will also be considered while performance evaluation.}

Extra Curricular Activities {Participation in extracurricular activities will also determine contribution towards organizational objectives}

Problem Solving and Decision Making Skills {Consistently demonstrate the ability to successfully move into problem-solving mode whenever challenges or concerns arise}  {Identify the purpose of decision, Brainstorm and list different possible choices, Put the decision into action & Evaluate the outcome of decision and action steps}
